Send the referral. See it arrive. Know when the report is back.
iReferral tracks a referral from the first request to the returned report. Every stop has an owner and a time, and the dentist who answers needs no account.

Times are made up. The point is the shape: each stop either confirms or shows who is waiting.
Step one
Find your hospital
Names and places come from the public CMS Hospital General Information file. A listing is not an endorsement. Nothing you type here leaves your browser.
Where adoption stands
Every hospital starts listed. Claimed and used are earned.
This is the public count, updated when a hospital claims a page or closes its first referral.
Built from public federal data. Nothing to sign.
The first systems to claim set the standard for the rest.
Used means one referral went through all six stops and the report came back.
A referral is not finished when it is sent. It is finished when the report is back with the office that asked.

Three people, one referral, the same six stops

Four questions to ask your care team, and a one page request to hand them and your dentist.

Three levels, and every hospital starts at the first
Listed
A page exists from public federal data. A dentist or physician can send a referral toward the hospital today.
Claimed
A named person at the hospital, often a quality director or care coordinator, owns the page and the intake contact.
Used
One referral reached all six stops and the report came back closed. Only then does a hospital show as used.
Start with your system
One decision can cover a whole health system
If your hospital belongs to a larger system, the system page lists every hospital in it. A system owner can claim them together.
Questions people ask
Plain answers
What is iReferral?
A free place to follow a referral from the first request to the returned report. Each of six stops has an owner and a time, so the office that asked can see where it is waiting.
Why is my hospital listed if it never signed up?
Every hospital in the public CMS file is listed so a referral has somewhere to go. Listing does not mean the hospital has joined or endorses anything. To correct or remove a listing, email eric@hsfm.co and it is done.
Does this site hold patient information?
No. This site is a directory and a set of guides. It has no forms, no accounts and no tracking. The referral form is a separate product with its own privacy terms.
Is it free?
Dentists and patients pay nothing. The referral form is free to use, and nobody pays to be listed or to be sent a referral.
What do Listed, Claimed and Used mean?
Listed means a page exists from public data. Claimed means a named person at the hospital owns the page. Used means one referral went through every stop and the report came back closed.
Can a patient start a referral here?
Not yet from this site. Today a patient can ask the care team to send one, or carry the one page request from the patients page to the care team and the dentist. A patient started referral is the next thing we are building.
Where does the hospital information come from?
The CMS Hospital General Information file for names, places and types, and the CMS Hospital Enrollment files for parent organizations. Both are public federal data.
Who runs it?
Eric Chong. Contact: eric@hsfm.co.
